Doctor बदला, Hospital बदला — लेकिन Medical History नहीं जाएगी
जब भी नए doctor के पास जाते हैं तो सबसे पहले यही कहते हैं — पुरानी reports लाओ, पुरानी prescriptions दिखाओ। अगर report घर पर है, prescription मिल नहीं रही, या पुराना hospital दूर है तो doctor बिना history के treatment शुरू करता है जो कभी-कभी dangerous हो सकता है। Ayushman Bharat Digital Mission यानी ABDM इसी problem को solve करता है। इसमें ABHA यानी Ayushman Bharat Health Account एक digital health ID है जिससे आपकी सारी medical history एक जगह store रहती है और किसी भी doctor को share की जा सकती है।
ABHA Health ID क्या है?
ABHA एक 14 digit का unique health identifier है। इसे Aadhar number या mobile number से बनाया जा सकता है। एक बार बन जाने पर सभी consultations, prescriptions, test reports, discharge summaries और vaccination records इस ID से linked हो जाते हैं। यह records digitally secure cloud पर store होते हैं जिन्हें सिर्फ account holder की consent से access किया जा सकता है।
ABHA ID कैसे बनाएं?
abha.abdm.gov.in पर जाएं या Aarogya Setu app open करें। “Create ABHA” option select करें। Aadhar based registration में Aadhar number डालें, OTP verify करें और profile complete करें। Mobile based registration भी possible है। ABHA ID बनते ही एक ABHA card download किया जा सकता है।
Digital Health Records कैसे Store होते हैं?
जब भी आप किसी ABDM integrated hospital या clinic में जाते हैं तो वो आपसे ABHA ID माँगते हैं। आप consent देते हैं तो वो उस visit की information आपके ABHA account में save कर देते हैं। PHR app यानी Personal Health Records app पर यह सब records देखे जा सकते हैं। आप किसी नए doctor को अपने ABHA account से selected records share कर सकते हैं बिना physical papers लाए।

PHR app play store पर available है। ABHA ID से login करें। यहाँ अपनी medications list maintain करें, doctor appointments track करें, test reports upload करें, और health vitals जैसे blood pressure, sugar, weight record करें। Emergency contact भी set करें।
ABDM Integrated Hospitals कहाँ हैं?
abdm.gov.in पर “ABDM Participating Health Facilities” section में अपने area के integrated hospitals और clinics देखें। जितने ज्यादा ABDM integrated facilities होंगी उतना ज्यादा आपकी digital health history automatically update होती रहेगी।
Privacy और Security का क्या?
ABHA account में सब कुछ encrypted होता है। कोई भी doctor या hospital बिना आपकी explicit consent के आपके records access नहीं कर सकता। Consent temporary हो सकती है जैसे सिर्फ एक consultation के लिए। आप कभी भी किसी doctor की access revoke कर सकते हैं। Account delete करने का option भी है।
एक जरूरी बात
ABHA Health ID बनाने से Ayushman Card नहीं मिलता — यह दो अलग चीजें हैं। ABHA एक digital health record system है जबकि Ayushman Card एक insurance scheme है। दोनों अलग-अलग register होते हैं। ABHA सभी के लिए free है जबकि Ayushman Card eligibility based है।
